Enrolment Forms and Medical Information

We ask everyone who attends a termly mat class, Personal Training or Sports Massage session to fill in an enrolment form. This asks you for name, address and phone-numbers and has a large health information section. It also asks you to agree to a list of terms and conditions.

To be able to do our job as Pilates teachers & Sports Therapists, we need to ask you information about health conditions and injuries. It is very important you give accurate and current information to us, and keep us up-to-date with your state of health.

Enrolment forms are stored as paper copies in a locked filing cabinet. They will never be uploaded.

We do not discuss teaching you or any details of your health with anyone outside of our group of Pilates teachers or Sports Therapists. We may only discuss your health between teachers when you are going to see another teacher or another teacher is covering the class.

We may ask your permission to talk or email your physiotherapist, consultant, doctor, or other health professional if we think this would benefit your treatment. You would be entitled to see this correspondence if requested.

"Physical fitness is the first requisite of happiness. In order to achieve happiness, it is imperative to gain mastery of your body. If at the age of 30 you are stiff and out of shape, you are old. If at 60 you are supple and strong then you are young." - Joseph Hubert Pilates
